A device that makes controlled punctures in the skin — simple in principle, and entirely dependent on how it is done.
A motorized or manual device drives fine needles into the skin to create controlled micro-injury. Devices differ, and some combine needling with radiofrequency energy, which puts them in the energy-device category with the thermal risks that come with it. Ask which type is being proposed and whether energy is involved.
The questions that matter most are unglamorous. Is the cartridge or needle head single-use and opened in front of you. How is the device handled between patients. What is being applied to the skin during and immediately after the treatment, and is that product intended for use on skin that has been opened — because a topical that is safe on intact skin is not automatically safe when it is being driven past the barrier. Who performs the treatment, what does their license permit at this depth, and who supervises them.
Expect redness and sensitivity afterward. The risks are infection, prolonged redness, scarring, and pigment change, again with higher pigment risk in darker skin tones. Active acne with infection, cold sores, open wounds, recent isotretinoin use and certain skin conditions are reasons to postpone or avoid it. If a practice cannot answer the sterility questions directly, that is a complete answer in itself.
